Continuing Legal Power of Attorney – This is the form adopted by Texas legislators to designate an agent authorized to take certain actions with respect to your property. The legal basis for this form is Texas Probate Code § 490. This form is available in Word or WordPerfect. Permanent Legal Power of Attorney with Informational Disclosure as modified by Glenn M. Karisch – This is not the basic legal form. Rather, it contains an informative disclosure and changes to the legal form developed by Glenn M. Karisch of Barnes & Karisch, P. C. This form is available in Word and WordPerfect. Medical Power of Attorney with Statement of Disclosure Required – This is the form issued by the Texas Legislature to appoint an officer to make medical decisions for you.

The legal basis for this form is the Texas Health and Safety Code §§ 166.163 and 166.164. This form is available in Word or WordPerfect. Guideline for Physicians and Surrogate or Family Mothers – This is the form passed by the Texas Legislature to indicate your wishes in case you are diagnosed with a terminal or irreversible illness. The legal basis for this form is Texas Health and Safety Code §166.033. This form is available in Word or WordPerfect. Guardian`s Declaration in Case of Subsequent Incapacity or Need for a Guardian – This is the form issued by the Texas Legislature to determine who you will serve as guardian (not guardian of your minor children – another form will be used for this) if you later become unable to work. The legal basis for this form is Texas Probate Code § 679. This form is available in Word or WordPerfect. Declaration of Guardian Appointment for My Children in the Event of Death or Incapacity for Work – This is the form enacted by the Texas Legislature to determine who you wish to have as guardian for your minor child or disabled adult child in case you later die or become unable to work. The legal basis for this form is Texas Probate Code § 677A.

This form is available in Word or WordPerfect. Non-parent Authorization Agreement – This tool was approved by legislators in 2009 when Chapter 34 of the Texas Family Code was enacted. (1) Form designed by Carish to be used by both parents in Word and PDF format; (2) Form developed by Karish for use by surviving parents when a parent has died, in Word and PDF format; and (3) Form published in PDF format by the Texas Department of Family and Protective Services. Mental Health Treatment Statement – This is the form passed by the Texas legislature to express your wishes regarding the granting or suspension of certain mental health treatments. Karisch`s article “Court-Created Trusts” for more information) (in WordPerfect format) Under the Succession Act 2007 Amendment Section 128A, Personal representatives must inform beneficiaries within 60 days of reviewing a will.
