Guideline for Physicians and Surrogate or Family Mothers – This is the form passed by the Texas Legislature to indicate your wishes in case you are diagnosed with a terminal or irreversible illness. Guardian`s Declaration in Case of Subsequent Incapacity or Need for a Guardian – This is the form issued by the Texas Legislature to determine who you will serve as guardian (not guardian of your minor children – another form will be used for this) if you later become unable to work. Declaration of Guardian Appointment for My Children in the Event of Death or Incapacity for Work – This is the form enacted by the Texas Legislature to determine who you wish to have as guardian for your minor child or disabled adult child in case you later die or become unable to work.
